Early rates reflect circumcision rates of males born in urban American hospitals in New York City, Philadelphia, and other major North Eastern cities. The majority of the American population has traditionally been rural rather than urban. Wallerstein's rates do not reflect a national average -- throughout most of the 1800s, and through the 1930s (until close to the time of WWII) neonatal circumcision in the rest of the United States was virtually unheard of.
