To witness infant genital cutting: to witness sexual assault

"It is virtually impossible to witness infant genital cutting and not recognize this act as a form of sexual assault." -Danelle Frisbie

The two most common forms of unnecessary infant genital cutting currently in practice in the United States include male circumcision by Gomco Clamp and male circumcision by Plastibell. If you've not previously witnessed how these surgeries take place, often without anesthesia, we invite you to research further, beginning with linked resources and videos below.

The Plastibell Lie

By Robin Vaughn-Bortolus © 2012

After four days of labor to bring my sweet son into the world, he arrived perfect.

I was exhausted when a nurse came in and said that it was time for him to be circumcised. I had been preparing for my sons arrival for months. I had read everything I could to try and make sure that I made informed decisions on his behalf. I chose breastfeeding, no vaccines at birth, etc. I really thought I had covered ALL the things I would need to be prepared for.

But there I was faced with a procedure that I had failed to educate myself on: circumcision.

I told the nurse that I was not okay with my son being cut. She informed me that this hospital used a no-cut device called a Plastibell. She said that it was a pain free method for my son. It was in that moment that I made a decision on his behalf that I regret, and will regret, for the rest of my life. I signed off on the procedure.

I knew that I wasn't okay with a scalpel, knife, etc., being used on my newborn -- but here was a great cut-free, pain-free alternative, right? I could not have been more wrong.

When my son was brought back to me he was crying in such a way that shattered my heart. I knew in that instant that I had made a terrible mistake. It was only after getting home that I looked up the Plastibell method and discovered the terrible truth behind it. I was horrified at my ignorance.

Two weeks later my son had his check up and the physician told me that the Plastibell procedure had not been performed correctly - that my son's foreskin was still "to long." Without so much as asking my permission my son's doctor had his nurse begin to gather the necessary instruments to "fix the problem."

In that moment, I informed the doctor that my son was fine as he was and that we would NOT be "fixing" anything. He did not approve and we were forced to find another doctor. I failed my son when I signed those permission papers in the first place, but I refused to damage him further.

I can never take back what I subjected my son to by allowing him to altered. But hopefully by sharing this I can encourage other parents to be more informed than I was, and to not fall for the Plastibell lie.

Patti Ramos: Circumcision Photos

Oh. my.

Patti, I love your work. You are a brilliant photographer, but how did you even make it through the sexual assault of this helpless newborn baby boy?

The terror in his face and tears in his eyes say it all.

People, THIS is what the genital mutilation of a newborn baby in the United States looks like. It will hurt you to look, but this little boy deserves his story to be told (even if only through the pictures captured of his abuse).

This baby was one of the 4% who received a small amount of lidocaine (96% of boys cut in the U.S. and Canada do NOT receive anesthesia prior to circumcision as it is counter-indicated for newborns). However, research shows that even anesthesia does not eliminate the excruciating pain (the penile dorsal nerve is unable to be blocked and the prepuce and frenulum are torn below this point). In addition, babies brains, nervous systems, respiratory, enzyme levels, stress hormones, and cardiovascular systems are STILL impaired as a result of genital cutting at a time they have extreme heightened sensitivity, even with anesthesia (Edell 2008; Fleiss & Hodges 2002; Reiss 2008).

Let's end it from happening to another - TODAY!

Massive Infection Takes Over Body After Plastibell Circumcision

This pediatric urology crisis occurred after a routine circumcision as a result of the Plastibell method -- one which some advertise as being 'less traumatic' than other methods of genital cutting. For more on the Plastibell method, see here.

PEDIATRIC UROLOGY

  • John H. Ngan, F.R.C.S.
  • Michael Mitchell, M.D.

"I think this child has an infected penis after neonatal circumcision..."

  • Authors: John H. Ngan M.D., J Waldhausen M.D., Richard Santucci M.D.

CASE REPORT

A 6 day old male was circumcised uneventfully using the Plastibell technique in the pediatrician's office. Two days later the patient developed penile shaft swelling and blistering accompanied by a temperature of 38.3 degree celsius. Intravenous Ampicillin and Gentamicin was started by the pediatrician. Gram stain of the blister aspirate revealed no inflammatory cell or organism. The patient improved the next day and antibiotics were changed to intramuscular ceftriaxone.

On post-circumcision day 4, urologic consultation was obtained as the penile swelling had extended into the scrotum and the penis started to blister. The Plastibell ring was removed without difficulty and the blisters drained. An ultrasound was obtained which revealed scrotal edema with normal testicles bilaterally. Provisional diagnosis of necrotizing fasciitis was made and the patient was transferred to Children’s Hospital Medical Center by helicopter.

On arrival, examination revealed a swollen scrotum with induration and erythema extending to the lower abdominal wall (See Figures One and Two below). A coin-sized patch of purplish discoloration was also evident on the right hemi-scrotum. The circumcision wound was covered with yellowish granulation tissue. His temperature was 37.6 deg Celsius, heart rate 160-170/min and blood pressure 80/60 mmHg. WBC was 27,000/cmm with 12744/cmm polys and 1296/cmm bands.

Fluid resuscitation with adminstration of broad spectrum antibiotics using vancomycin, gentamicin and clindamycin. The child was brought into the operating room for debridement of necrotizing fasciitis. The infective process had involved the upper thighs, the lower abdominal wall and posteriorly to the perineal body. Wide excision of scrotum, penile skin, and lower abdominal wall was carried out. Deep tissue layers including the penile shaft, testicles, and abdominal musculature appeared normal. (See Figure Three)

Figure Three

Post-operatively, the child underwent second-look surgery and further debridement to the level of the mid-abdomen and the superior thigh on post-operative day 2. (See Figure Four)

In addition, 7 hyperbaric oxygen treatments were given. On post-operative 15, split-thickness skin autografts were applied to the lower abdomen, penile shaft, thighs and testicles.

DISCUSSION

Technique of Neonatal Circumcision.

Neonatal circmucisions are commonly performed by obstetricians and pediatricians in the first week of life in their offices in Washington. In general, either the plastibell or the Gomco clamp techniques are used. While both techniques should give reasonable cosmetic results, the plastibell alows better visualization of the circumcision. With the plastibell, a ring is left to assist coaptation of the wound edges after the circumcision. Upon healing, the ring will be released from the wound. The Gomco clamp leaves no foreign body on the wound but has the risk of wound edge separation. Moreover, the Gomco clamp does not allow an accurate estimation of skin to be excised and can remove excess skin when the location of the penoscrotal junction is misjudged. It is important that electro-coagulation (in place of the scalple) is not used on the Gomco clamp during circumcision. The Moyal clamp performs the circumcision on the same principle as the Gomco clamp. There is no device to hold the wound edges together. Most pediatric urologists prefer using the free-hand technique.

Whether the type of techniques used is related to a higher incidence of infection is not entirely clear. Gee and Ansell compared the use of Plastibell and Gomco in 5521 children between 1963 and 1972. In the Plastibell group, 19/2625 (0.72%) infections encountered. In the Gomco group, 4/2896 (0.14%) circumcisions infected. The incidence of infection following Plastibell circumcision was significantly more frequent than that following Gomco circumcision (p<.005). Moreover, 4/19 Plastibell infections developed irritability and required antibiotic treatment while all Gomco infections were successfully treated with topical agents. The other patient with Fourniew's gangrene developed after circumcision reported in teh literature also had a Plastibell circumcision (Woodside JR, 1990). Of all the serious infections that are known at this center, the plastibell has been used.
Care after circumcision.
The wound more often than not will look edematous and even red. However, the erythema should not spread to penile shaft skin. Erythema in the shaft skin should raise the concern for spreading wound infection and impending faciitis if not treated promptly. Though WBC in the neonates can be as high as 30,000/mm3, neonates with faciitis in general will demonstrate leukocytosis with bandemia and WBC higher than 20,000/mm3. Normal neonates do not show bandemia. Admission for observation and broad spectrum antibiotics is warranted. However, there is no consensus on whether the plastibell should be removed at this point. The plastibell is an external device and it is not unreasonable to leave it on if the circumcision is only recently performed. Surgical intervention is indicated when faciitis is suspected.
Surgical management.
High index of suspicion, prompt diagnosis, immediate fluid resuscitation and institution of broad spectrum antibiotics followed by debridement is the key successful treatment of Fournier's gangrene. The disease is extremely lethal. In adults, the recent mortality has improved to 18% (Clayton et al, 1990). In children, the overall mortality rate is 9% and can be as high as 30% in infants younger than 3 month (Adams JR et al, 1990), the time when neonatal circumcision is performed. Faciitis caused by streptococcal infection and Chicken pox on the face has been successfully treated by raising skin flaps for subcutaneous drainage without excision. In this way, skin can be preserved and cosmesis preserved. At present this technique should not be extrapolate to use in Fournier’s gangrene which is polymicrobial in nature. Wide excision should be the rule followed by frequent re-examination.
It is important that the testes should be preserved as necrotising faciitis almost never involves the testes. The tunica vaginalis should remain intact during dissection so theat the testes will not dessicate. Retraction of testes occur secondary to wound contraction. Tagging the testes in their orthotopic position after debridement may decrease the extent of proximal migration.

MRI Studies: The Brain Permanently Altered From Infant Circumcision

By Dr. Paul D. Tinari © 2008

Two of my physics professors at Queen's University (Dr. Stewart and Dr. McKee) were the original developers of Positron Emission Tomography (PET) for medical applications. They and a number of other Queen's physicists also worked on improving the accuracy of fMRI for observing metabolic activity within the human body.

As a graduate student working in the Department of Epidemiology in 1998, I was approached by a group of nurses who were attempting to organize a protest against male infant circumcision in Kinston General Hospital. They said that their observations indicated that babies undergoing the procedure were subjected to significant and inhumane levels of pain that subsequently adversely affected their behaviours. They said that they needed some scientific support for their position. It was my idea to use fMRI and/or PET scanning to directly observe the effects of circumcision on the infant brain.

The operator of the MRI machine in the hospital was a friend of mine and he agreed to allow us to use the machine for research after normal operational hours. We also found a nurse who was under intense pressure by her husband to have her newborn son circumcised and she was willing to have her son to be the subject of the study. Her goal was to provide scientific information that would eventually be used to ban male infant circumcision. Since no permission of the ethics committee was required to perform any routine male infant circumcision, we did not feel it was necessary to seek any permission to carry out this study.

We tightly strapped an infant to a traditional plastic "circumrestraint" using Velcro restraints. We also completely immobilized the infant's head using standard surgical tape. The entire apparatus was then introduced into the MRI chamber. Since no metal objects could be used because of the high magnetic fields, the doctor who performed the surgery used a plastic bell ("Plastibell") with a sterilized obsidian bade to cut the foreskin. No anaesthetic was used.

The baby was kept in the machine for several minutes to generate baseline data of the normal metabolic activity in the brain. This was used to compare to the data gathered during and after the surgery. Analysis of the MRI data indicated that the surgery subjected the infant to significant trauma. The greatest changes occurred in the limbic system concentrating in the amygdala and in the frontal and temporal lobes.

A neurologist who saw the results to postulated that the data indicated that circumcision affected most intensely the portions of the victim's brain associated with reasoning, perception and emotions. Follow up tests on the infant one day, one week and one month after the surgery indicated that the child's brain never returned to its baseline configuration. In other words, the evidence generated by this research indicated that the brain of the circumcised infant was permanently changed by the surgery.

Our problems began when we attempted to publish our findings in the open medical literature. All of the participants in the research including myself were called before the hospital discipline committee and were severely reprimanded. We were told that while male circumcision was legal under all circumstances in Canada, any attempt to study the adverse effects of circumcision was strictly prohibited by the ethical regulations. Not only could we not publish the results of our research, but we also had to destroy all of our results. If we refused to comply, we were all threatened with immediate dismissal and legal action.

I would encourage anyone with access to fMRI and /or PET scanning machines to repeat our research as described above, confirm our results, and then publish the results in the open literature.

Here we go again: New York Times publishes headline on HIV & Circumcision

Unless you are living under a rock -- or in a much kinder, secluded world someplace -- you have noticed the pop media articles discussing all the 'research' we are doing in Africa involving HIV and African's penises (yes, another sexually transmitted infection study conducted by white, American men in lab coats on black, unsuspecting persons from a foreign country).

Companies, corporations and groups that benefit from the surgical removal of the prepuce organ and/or benefit from the rampant sale of foreskins, back this problematic 'study' that continues to be spread across U.S. headlines like candy for our circumfetish taste buds.

Gomco Circumcision Clamp

The latest pop media article appeared today in the New York Times. Although this article is careful to state that, "There is little to no evidence that circumcision protects men who have sex with men from infection," it steers the reader to believe that circumcised men who have sex with women are protected from infection. Nothing could be further from the truth.

The Times article also states that 'observational studies' indicate intact men have higher rates of sexually transmitted infections (STIs), yet neglects to mention the past 30 years of empirical, peer-reviewed research conducted world-wide in the areas of human sexuality, health, and disease that show exactly the opposite to be true. In fact, intact men living in developed nations have much lower rates of all sexually transmitted infections, HIV included.

One of the primary purposes of the prepuce organ (foreskin) is to naturally protect, clean, and kill foreign invaders with its own special concoction of antibodies. Remove the prepuce, and this most excellent source of protection is gone. It has been repeatedly demonstrated that circumcised men (in all countries) have higher rates of all STIs (including HIV) across the board. Intact men certainly are at a benefit as far as health is concerned.

That being said, the U.S. has (by far) the highest rate of circumcised adult men of any Western nation. We circumcise more baby boys (currently about 32% of newborns) than any other nation on earth (far more than any other country, as Canada has the next highest circumcision rate of only 10%). The United States also has the highest rate of all sexually transmitted infections (including HIV) of any developed nation. Western nations where 98-99% of their boys/men remain intact have the lowest rates of STIs (including HIV). If circumcision 'protected' against diseases we would not see these figures to such an extreme and obvious degree.

Mogen Circumcision Clamp

Despite all of this, why are we focusing so much on cutting/not cutting genitals when it comes to STIs and HIV?! The only real protection comes from (1) abstaining from sex w/ infected partners or (2) condom use. We already know these things. Did we forget? Maybe we should pour our resources into condom use/education in South Africa and here on the home front rather than trying to cut everyone up...Instead of spending any more energy, time and money hashing out whether or not we should chop up baby boy's penises at birth, we should concentrate on methods that actually DO prevent the spread of sexually transmitted diseases, such as condom education and dispersal.

Plastibell Circumcision Clamp

In addition to the mis-information portrayed by the African/HIV/circumcision study behind this Times article, countless medical groups from around the world have critiqued and called into question the methods used for this study. When first "results" showed no difference between cut vs. uncut men, another small sample of African men was 'recruited' and divided ahead of time into those who tested positive vs. negative for HIV, and those who were intact vs. circumcised, so the sample could be an accurate picture of what they were hoping to show. Although the whole thing is bogus to begin with -- even if it were valid, it has nothing to do with the routine infant circumcision of baby boys in the United States.

The Times article also mentions the commonly thrown around belief that circumcision reduces rates of urinary tract infections in baby boys (under the age of 1). However, UTIs among boys/men are very rare to begin with, and when they do occur, are almost always due to a well-meaning, but misinformed adult 'messing' with, forcibly retracting, or over-cleaning a baby boy's penis and foreskin. Women and girls have a 900x greater likelihood of getting a UTI than their male counterparts. Should we cut off the prepuce organ from all baby girls at birth as well to protect them from their real risk of a UTI? Obviously they would 'benefit' more than the boys do... No, of course not. Because UTIs are easy to prevent with proper information, and when they do occur, they are minor and EASILY and quickly treated with antibiotics.

Fortunately more and more physicians who are knowledgeable and well-educated on this topic are speaking up and loudly saying "NO" to the practice: DoctorsOpposingCircumcision.org.

Doctors and other health care specialists are openly taking a stand and dispersing the myths and misconceptions behind the genital cutting practice in the United States. In popular media shows like Penn & Teller, as well as in more research-based educational films such as Cut: Slicing Through the Myths of Circumcision and The Circumcision Decision, we are reminded that circumcision is a "solution" in search of a problem. We have tried (for almost a century now) to come up with an excuse for our rampant chopping of baby penises. First, Kellogg and Graham told the country that we could decrease masturbation (1940s) by circumcising all boys. We would prevent blindness and other ills. It would curtail men's sexual exploits and we would see a decrease in birth out of wedlock (1950s). We believed it would cure and prevent cancer (1960s) and make a significant difference in urinary tract infections, and even cancer among women. We just keep trying to come up with something - anything - to justify why the United States is the only country in the world genitally cutting 50% of newborn baby boys.

There are reasons -- and quite a few actually -- that no medical or health organization in the world recommends infant circumcision. There are reasons that more and more religious Jews today are electing to have a Brit Shalom in place of circumcising their son. There are reasons that those in the Christian faith are becoming educated on what the Bible, specifically Jesus in the New Testament, says about circumcision, before subjecting their sons to genital cutting. There are reasons that Australia is on the brink of joining several other countries (Ireland, Sweden, Denmark, Netherlands, Finland, Germany, among other European nations) in banning all forms of genital mutilation on baby boys or girls and making it illegal for doctors to amputate the prepuce organ from non-consenting persons.

Sheldon Circumcision Clamp

The most problematic aspect of the argument stirring this Times article and the select pro-circ groups attempting to lobby the CDC into advocating for routine infant circumcision is the severe violation of human rights that occurs when a healthy, functioning organ is permanently removed from a non-consenting person. Even though we know that circumcision does not prevent HIV - if it DID -- even if it prevented ALL HIV -- would we then be justified in taking away a man's personal genital integrity by forcibly amputating his prepuce as an infant? Forever taking away HIS choice in the matter.

We could very well amputate ALL breasts from all girls and witness the disappearance of breast cancer before our eyes. After all, breast cancer inflicts FAR more women in this country than HIV ever has.

Should we then do so?

This argument is not only completely absurd, but entirely misguided.

Hopefully, when it comes to basic human rights and genital integrity, the United States will introduce the MGM Bill along side the FGM Bill and follow suit with the rest of the educated world in recognizing that we have no business mixing knives and babies' penises. We are all about freedom of choice here, right? Well, it is HIS body, HIS penis - and it certainly should be HIS choice.

Until your adult, grown son elects to travel to Africa on his own accord and engage in unprotected, sexual intercourse with an HIV positive, African man, and chooses for himself to amputate his prepuce organ ahead of time in hopeful preparation for his ventures -- LEAVE HIM INTACT.

Plastibell Infant Circumcision

The Plastibell comes in six sizes to fit over the glans of various baby boys' penises at birth. The most common size used is 1.3cm in diameter, followed by 1.2cm. Larger sizes of 1.5 and 1.7cm are used infrequently.

It is terribly troubling that a lot of parents are told that the Plastibell form of circumcision is "less painful" or "less traumatic" for a newborn. For this surgery, a Plastibell is used rather than a Gomco Clamp or Mogen Clamp. "Plastibell" may sound like a more inviting tool to use on a highly sensitive penis than "Clamp." The reality, however, is that all forms of genital cutting are equally excruciating for a newborn. All forms amputate the purposeful prepuce organ which serves many important functions in infancy, childhood, and adulthood. If you are considering having your baby circumcised using the Plastibell method, here are medical professionals carrying out the surgery so you are able to know what is involved and make an informed decision.

Note that the first video was made by a pro-cutting doctor and his assistant. The baby's screaming is muted and words are placed on the screen describing what is being done. Not all of the information is entirely accurate -- NO health or medical organization in the entire world recommends circumcision on infants.

In addition, no health or medical organization in the world recommends that anyone other than the young boy himself 'mess with' his foreskin. While the average age of retraction is 10.6 years of age, sometimes this natural retraction does not occur until the hormones of puberty naturally loosen the prepuce. The problems that have arisen for a minority of intact babies, boys, or men, in the U.S. occur because someone forcibly retracted or attempted to 'clean' between their foreskin and penis glans (head) before it was retracting fully on its own. This is the same as if we pulled back the fingernails of a newborn baby to 'clean' between their fingernails and fingers, or if we scrubbed out the area between the eyelids and eyeballs. The prepuce is tightly adhered to the glans (head) of the penis. There is zero cleaning care needed for an intact baby boy. The prepuce is a self-cleaning, self-sustaining, remarkable and necessary organ when it is left alone on the body of a child.

Additional video examples of Plastibell circumcision surgery:

For promotion of his practice, this Australian physician (who profits on the sale of circumcision surgery) uses a large amount of anesthesia - both topically before injection, and local anesthesia in three locations, pressing deep into the penile tissues to reach the dorsal nerve running to the base of the frenulum. He also waits adequate amount of time for the anesthesia to kick in. Rarely is this practice (either the heavy amount of anesthesia, placement, or time in waiting for it to take effect) carried through as such in the United States or Canada. This video is a rare one in which the baby has adequate local anesthesia to dull the pain of the surgery, and the process - the amputation of the prepuce by Plastibell - can be seen clearly. Unfortunately, the physician fails to discuss the monumental pain that will ensue when this local anesthesia wears off, or all the many purposes of the prepuce organ that have now been forever removed from this baby. Anesthesia is counter-indicated for newborns by the American Academy of Pediatrics, which is one reason the majority of hospitals conducting genital cutting do not utilize it in their newborn circumcision surgeries. Another reason, as one U.S. physician recently stated is because, "I can get the job done much faster without having to wait 10 minutes for anesthesia to kick in."

Aside from the Plastibell, the Gomco is the other most commonly performed method of circumcision in the United States. When a parent decides against keeping their child intact, one of these two methods of circumcision surgery will almost always take place in the North America. As a result, we include one example of the Gomco here, and further information can be found at this resource page: http://www.drmomma.org/2011/01/neonatal-circumcision-video-for.html

Baby Dies Post Plastibell Circumcision

From CanWest News Service: June 13, 2007, Ottawa, Canada.

A one-week-old Ontario infant died from complications after undergoing a circumcision in a provincial hospital. Information about the case was published in the April edition of Paediatric Child Health. The baby, whose name has been withheld by the parents, passed away after his kidneys became enlarged to seven times their normal size. The child was born at an unidentified Ontario hospital “sometime in the last three years,” said Dr. Jim Cairns, Ontario’s deputy chief coroner. “The family wants to keep this anonymous.” No charges were ever laid and no legal action was ever taken in the case. According to the Paediatric Child Health article, the boy was “bottlefed and was reported to be doing well when he was circumcised.” Five hours later, the parents returned to their family doctor with the infant, who had become “irritable and had blue discolouration” below the belly button. Doctors noticed the discolouration and slight swelling of the penis, but sent the child home.

Fourteen hours after the circumcision, according to Cairns, the child was brought to another hospital where doctors noted he was extremely irritable with marked swelling of the penis and bruising to the scrotum. The child was then transferred to a paediatric centre, where his bladder was diagnosed, Cairns said, to “seven or eight times its normal size.”

The PlastiBell ring, which is used to hold back the foreskin after circumcision, was removed and drained and the child went into shock. “If the Plastibell had been taken off five hours after he got there, he would be alive,” said Cairns.

The child’s death was attributed to septic shock — “an overwhelming infection, leading to multi-organ failure,” Cairns said. “Death is rare after circumcision,” said Cairns. “But complications can happen.” The case was brought to Cairns’ attention because the circumstances of every death of an Ontario child under five years of age must be reviewed by the provincial coroner’s office.
